There is no substitute for the Mark I eyeball.  Using your navigational instrumentation alone is not enough to guarantee safe navigation.  This case study includes a reenactment and analysis of an incident in which poor communications on the bridge and a lack of attentiveness lead to a collision between a tanker and a car ferry.
